What is the charge for the initial evaluation and for treatment?

The initial evaluation is an extended visit (usually 90minutes, but the duration may vary). This allows sufficient time for a thorough evaluation, answering questions, and getting started with treatment all in the first visit. The charge for the evaluation is $280. Subsequent treatment sessions are about an hour and are $140/hour. 30 minute sessions may be available upon request and are $75.00. Payment is due at the time services are provided.

Do you accept my insurance?

Complete Care Physical Therapy accepts both No-Fault and Worker’s Compensation. We also accept checks, credit cards (Visa, AMEX, Mastercard, or Discover), or cash for payment.

If you have other insurance coverage, we will work with you to help maximize your reimbursement as we are considered an "out of network" provider. You may be able to find out what your "out of network" benefits are in advance by contacting your insurance company. If you wish to seek reimbursement through private health insurance we will gladly provide you with a completed claim form.
