You may schedule an office visit either by phone or by using the online appointment request form below. To schedule an office visit by phone dial: 631-670-7033 All appointment requests will be confirmed by return e-mail or phone within 24 hours. When you schedule your initial appointment we will ask you for an email address or fax number to send you all of the necessary paperwork in advance of your visit. Please complete the paperwork BEFORE you arrive at our office. This way the time allotted for your appointment can be fully dedicated to one on one care and not to filling out forms. Please arrive a few minutes before your scheduled appointment time as our office runs on schedule and will not keep you waiting. We know your time is valuable. |
